A case study requires you to demonstrate skills and knowledge in relation to the management of a patient and their health needs in your professional field. It includes analysis and problem solving and is required to demonstrate evidence-based ethical professional practice.
A synthesis and integration of relevant literature to support your critical discussion is required in relation to contemporary evidence-based ethical professional practice.
Case Study to develop:
Anne a 86 year old female with dementia who comes to the GP surgery to get her diabetic foot ulcer dressed twice a week by the practice nurse.
Scenario: Anne is a patient who suffers from dementia and is regularly missing her appointments to get her wound dressed. Anne is obese and has type 2 diabetes mellitus.
Anne is accompanied to her dressing appointments by her husband John who is old and frail.
Anne has had the following appointments for her diabetic foot ulcer dressings:
1st appointment: Anne attended with husband John for diabetic foot ulcer dressing.
2nd appointment: Did not attend appointment
3rd appointment: Husband John expresses his concerns about Anneâs decline in mental health and memory- John reveals he is not able to cope on his own as he too is old and frail.
Full assessment of Anne is carried out- ABCDE assessment, MUST screening tool, Body mass index (BMI)
Priority health need: Anne is obese with a BMI of 30, high HbA1c levels. Anne is also forgetting more now, forgot she had an appointment with the practice nurse to get dressing of diabetic foot ulcer. Husband John is now struggling to cope as he is old and frail.
Multi-disciplinary team that needs to be involved: Diabetes specialist nurse, dietician, general practioner and advanced nurse practioner.
Patients needs: the goal is to manage hyperglycaemia to prevent the progression of the diabetic foot ulcer, maintain a healthy weight and provide husband John with support in looking after Anne.
Your case study should be developed under the following headings, and be written in the third person:
Introduction: In this section, you should briefly introduce your professional role as a student general practice nurse, your professional responsibilities in relation to confidentiality and anonymity, core contextual information pertaining to the patient, family and/or carer and the area/s and/or aspect/s of care management the case study is going to be focused upon.
Assessment: You should concisely summarise the comprehensive holistic assessment of the patient and any family/carer needs. You should cover nursing, social and medical history, any physical examination and investigation findings, family and carer roles, and be explicit in relation to any safeguarding needs. From this identify the priorities in relation to care management. It is the priority need/s which should form the focus of your case study. It is important that you provide a rationale as to why you have chosen to focus on this/these area/s aspect/s of care management and provide an evaluation of the literature used to support your critical discussion of the patient assessment.
Patient need/s: this section title will depend on the case management aspect you are focusing on. Do not discuss the patient but provide critical evaluation and analysis of the primary and wider evidence base for the management of care in relation to the focus of your assignment.
Patient Management: (you will require several paragraphs to develop the critical discussion, it will be important that this is linked to the evidence you have highlighted in the patient need section, you may wish to include further subheadings): in this section provide critical discussion and analysis on how the patient care was managed. You are also required to provide details of the care planned and how it was implemented, including if any other members of the multi-disciplinary/agency team were involved. Your discussion should be supported by reference to and analysis of current relevant healthcare/professional policy, guidance, strategy, and the wider evidence base.
Evaluation of Care: in this section, you should review and provide an evaluation in relation to the care interventions planned, including if they achieved the desired outcome.
Implications for future practice: based on your critical evaluation you should consider implications/recommendations for future practice, this can include current and emerging national and international evidence.
Conclusion: your conclusion should provide a succinct synopsis of the content of your assignment (you should not introduce anything new in this final section).
References: a reference list should be included in relation to all literature cited in the text of your body. We expect to see a minimum of 25-30 references included.
