DSM-V Diagnosis
Explanation of Diagnosis
Onset of symptoms
Symptoms client is showing for counseling to determine diagnosis
Any additional information applicable to client
The report should be constructed in a Word document; single or double spaced is allowed; can include the above mentioned points as section headers. Be sure to include the client’s name.
