I. Introduction

B. Discuss the background and significance of the problem to nursing. Be sure to substantiate your claims with specific examples from research.
II. Justification of Practice Change Recommendation
A. Provide a scholarly summary of the research reviewed. (Literature Review)

III. Pre-Implementation Plan
A. Identify a specific change theory that will assist you with planning stages of your change proposal Instructor’s notes: Describe the theory and why this theory will best help you implement your change proposal. How will using this theory work as you roll out your project?
B. Identify activities needed prior to implementing your change, making theory-to-practice connections with each key element and impact of change to the element
C. Discuss a plan for activities needed to leverage this element to promote change and identify the stakeholders that may be involved in these activities. The activities you propose must cover multiple aspects of health practice surrounding the problem, including but not limited to:
a. Patient and Nurse Safety Instructor’s notes: Describe specific patient and/or nurse safety issues that are affected with this change. Or, discuss what safety issues would be worsened without the change.
b. Information Management Systems Instructor’s notes: Describe appropriate use of technology related to the change. Identify other tools to support effective patient care related to the change.
c. Health Literacy Instructor’s notes: What health education for patients will be required with the proposed change? What clinical education for nurses will be required with the proposed change?
d. Administrative Policies Instructor’s notes: Discuss any current administrative or other policies (related directly or indirectly to your change) that need to be changed or put into place. Discuss WHY any administrative/policy changes need to be different or WHY NOT. Be specific.
e. *Ethical Considerations (i.e., equity of access and treatment, etc.) Instructor’s notes: Describe any procedures to guarantee equitable access to healthcare/treatment/prevention for patients across diverse populations and age groups.
* Think about whether there are any equity considerations involved for your problem or your particular population (patient, nurse, etc.).
If dealing with an issue involving patients, you need to consider population health concerns to address equitable access/healthcare or treatment/prevention.
Issues involving nurses’ equitable access/treatment/prevention may still apply depending on the situation so think it through or ask for suggestions in the small group collaboration.
