CASE STUDY Onyeka is a 27 year old female of Nigerian heritage. She has just been admitted to your medical ward from A+E with sepsis following a UTI. Onyeka is a university student studying physiotherapy. She is also a single mother to her 2-year-old child. Onyeka’s mother lives with them and helps with childcare but she has multiple co-morbidities and had a stroke last year, so Onyeka also supports her with health needs. As well as studying, Onyeka works part time as a clinical support worker to fund her education and support her family. She is often up very late at night trying to complete assignments. She is so stressed about financial worries she often bursts into tears and has withdrawn from friends as she cannot afford to socialise, nor does she feel she has the time. Onyeka explains that she has been suffering with a UTI over the past week, possibly due to dehydration as she’s so busy she forgets to drink in the day. She had not got round to getting antibiotics and her abdominal pain has been increasing over the past couple of days, causing her mother called an ambulance. Onyeka has Type 1 diabetes which is well controlled with insulin. On arrival the patient’s observations at were, HR – 125 RR – 29 BP –101/65 02 sats – 96% on 4L via face mask Temperature – GCS – E -4, V-5, M – 6 = 15. Pain 8/10 Onyeka has not passed urine with you but expresses that she went several hours ago, and it was very dark and smelly. On touch, her hands feel cold but her chest is warm.
