PROBLEM STATEMENT: Uncontrolled hypertension is an elevated high blood pressure characterized by a systolic blood pressure of at least 140mmHg and diastolic blood pressure of at least 90mmHg. 55% of patients 65 and older have a diagnosis of uncontrolled hypertension (Blood pressure-BP- 140/90) at Jessie Trice Community Health System.
How do you identify what outcomes you need to measure?
Project methodology includes measuring outcomes to identify the project’s effectiveness. How do you know if the changes you are making are leading to improvement? Simple – you measure. Measurement does not have to be difficult or time-consuming.
The key is to pick the right measures so that the quality improvement team can see results quickly and are able to adapt their interventions, accordingly, putting less strain on resources and more focus on outcomes.
