Remote Management of Uncontrolled High Blood Pressure for Elderly Adult Patients
PROBLEM STATEMENT: Uncontrolled hypertension is an elevated high blood pressure characterized by a systolic blood pressure of at least 140mmHg and diastolic blood pressure of at least 90mmHg. 55% of patients 65 and older have a diagnosis of uncontrolled hypertension (Blood pressure-BP- 140/90) at Jessie Trice Community Health System.
How do you identify what outcomes you need to measure?
Project methodology includes measuring outcomes to identify the project’s effectiveness. How do you know if the changes you are making are leading to improvement? Simple – you measure. Measurement does not have to be difficult or time-consuming.
The key is to pick the right measures so that the quality improvement team can see results quickly and are able to adapt their interventions, accordingly, putting less strain on resources and more focus on outcomes. What will be measured, how often will it be measured, who will be responsible for measurement, and how will the measurements be shared with the team, leadership, and the organization. Think of the outcomes you are planning to identify.
For the purpose of this ASSIGNMENT, determine what you will measure to determine the success of the quality improvement project.
Answer and elaborate on the following questions:
1. How do your outcomes support your project goals?
2. What are the benchmarks for your outcome measurements? evaluate the effects of remote BP monitoring in patients diagnosed with hypertension over a eight-week timeframe to see if there were improved outcomes with BP at or below 140/90 mmHg
3. What is your plan for measuring your outcomes? (i.e., surveys, questionnaires, other tools, data collection procedures, analysis, etc.): Use of surveys and questionnaires (breakdown how you will utilize these measures
4. Do these measures provide a picture of how well your goals were met?
Please note:
• Establish already proven evidence-based measures from the literature (ie. structure, process, outcomes) to evaluate the outcomes of the quality improvement project.
• Develop a plan for measuring the outcomes of a quality improvement project and how the data will be collected, displayed, etc.
• Discuss how the selected measures will determine if the desired goals were met; and to
• Devise a plan for how the outcomes will be disseminated to the stakeholders (ie. team, leadership, organization, community).
