Reaffirm the Problem Statement After using various brainstorming tools such as the flowchart, reaffirm the problem by rewriting your problem statement in 1 to 2 sentences. Then, describe the problem. Provide a national context for your identified problem by describing how it is linked to a national quality or safety indicator (or indicators). Aim: What are you trying to accomplish? Address all of the following items: What is the opportunity for improvement? Make sure it relates to a fundamental stakeholder need. How much will you improve? Are the parts/steps in the system performing as planned? List the measures that will tell you whether the steps or parts of the process are performing as planned to affect the outcome. Balancing measures: Look at the system from a different perspective or direction. List the measures that will tell you whether you are introducing problems elsewhere in the system. Plan to collect data: Write an initial plan for data collection. Who would be responsible for collecting data?
