Project proposal to prevent lead hazard in underserved communities
I.    2 pages:
    Provide a general background for a grant-based non-profit organization and description of the program plan and intervention.
    Describe how a non-profit agency (e.g., 501C3, board of directors) will affect the program planning for your Applied Project?
    Analyze the major socio-ecological factors affecting the application of the project and health of the population.
II.    5 pages:
    Identify a problem statement for the topic analyzing/annotating at least 10 recent (from 2019 up to date) primary peer-reviewed journal articles.
    For each study article, provide a brief description of the study aim/objective, the methods, the major findings and how you plan to use each finding.
    Synthesize the literature on the needs, assets, and interventions pertaining to the applied project.
    Based on the literatures, describe the rationale of the proposed program or intervention, and how the project will address the problem and help fill the knowledge gap.
III.    5 pages:
    Describe essential activities, process and outcome objectives of the project proposal
    Explain how each objective is a SMART objective
    Explain the method used in the intervention program and why you found it is to be the most appropriate methodology.
    Explain which type (outcome or impact) evaluation the project will use? Why this most appropriate evaluation type?
    Explain any ethical concerns inherent in the  methodology, and describe any formative, process, outcome or impact evaluation methods that needs to be used
    Explain how the evaluation is aligned to the intervention project and program competencies?
IV.    2 Pages
    Using the Dissemination Plan Worksheet (with standard template), create Dissemination Plan and Timeline for the project including Dates, Goals, Objectives, Major milestones, and Timeline.
    Justify your Dissemination Plan and the Timeline
V.    2 pages
    Create an optimum budget for your project justifying each line item.
    Describe Project Sustainability Plan for its continuation after the initial project period.
VI.    2 Pages
    Analysis of the project’s social change impact and explain how the proposed intervention helps to address Essential Public Health Services.
