You have been referred four clients who engage in trichotillomania. All of the clients have had comprehensive medical evaluations and behavioral interventions were recommended. The results of a functional analysis indicated that the behavior for all clients is likely to be maintained by automatic reinforcement. All of the clients have average cognitive abilities, are able to communicate vocally, and have family support. You have chosen to implement simplified habit reversal. Below is additional client information:
Gabriella is a 16-year-old Hispanic student who has a 3-inch bald spot on the back of the head. Gabriella has engaged in trichotillomania for approximately three years and reports that they often do not realize that they are pulling hair until Gabriella’s family points it out.
DeJuan is a 10-year-old African American student who initially pulled hair from their head. After the parent began having DeJuan’s head shaved, they started pulling out arm hair. DeJuan most commonly pulls hair at school when presented with frustrating task demands.
Todd is an 18-year-old Caucasian student who has engaged in trichotillomania for approximately 10 years. They pull hair from their head, eyebrows, and pubic region.
Cassidy is an 8-year-old African American student who recently began engaging in trichotillomania. At your initial appointment, you realized that they have removed all of their eyebrows and a majority of their eyelashes.
2. You are consulting in a large urban school district where you are responsible for training six resource teachers to implement Tier 2 and Tier 3 academic interventions with treatment fidelity. You recently facilitated a workshop where you trained the teachers to implement a math intervention to increase fluency and accuracy of single digit math facts. The intervention consists of “drill and practice” with an error correction procedure in which students say the correct facts three times if and error is made. During the workshop, you provided didactic instruction where you reviewed all components of the intervention. You provided each of the teachers with a scriipt that had each of the intervention components listed as well as instructions that should be read verbatim. After the training you observed each of the teachers implementing the math intervention and recorded the percentage of correct steps implemented.
3. You have been referred four typically developing clients from a Head Start classroom. Each of the clients presents with age-appropriate receptive and expressive language skills. Chris (4 y/o, Caucasian, student), Adam (3 y/o, Hispanic, student), Merritt (5 y/o, Caucasian, student), and Todd (6 y/o, African-American, student) were referred by their teachers because of problems with transitions and being off-task. A preliminary functional analysis suggests that the behavior is likely maintained by escape from a task. You have decided to implement visual activity schedules to address these referral concerns.
4. You have been referred a 9 year-old-student (Landon) with severe intellectual disability. Landon is nonverbal, but has some receptive language abilities. Landon’s parents currently keep Landon in protective undergarments because Landon has frequent toileting accidents. Recently, Landon has started to notify their parents that they have soiled their protective undergarments by going to their parents, tapping on their genitals, and whining. He has also started to demonstrate the ability to hold their bladder and bowels for extended periods of time. There are no known medical issues and the family has decided that they would like to attempt toilet training. The behavior analyst recently read an article suggesting that toilet training is most successful when the individual wears underwear. The behavior analyst will recommend the use of underwear, frequent access to liquid, and frequent trips to the bathroom as part of a toilet training protocol. Toileting successes will be recorded.
5. You have been referred a 9-year-old child (Asher) who engages in toe walking. Asher has toe walked ever since began walking. They are able to walk normally if instructed to, but their natural walking pattern includes toe walking. Asher participates in stretching activities with a physical therapist for 30 minutes, 5 days a week. After 3 months of physical therapy, no improvement has been made. Asher’s pediatrician has suggested behavioral interventions. The behavior analysts overseeing the case will use Gait Spots as a way to decrease toe walking.
