Complete and accurate medical recordkeeping and documentation can ensure that patients get the right care at the right time. When a nurse work the entire shift without documentation, it literally mean that, nothing was being done because, ”what is not written is not done”. Good documentation is important to protect the provider, avoiding liability and keep out of fraud and abuse of trouble, (US Department of Health & Human Sciences, 2022). Complete and accurate documentation has proven to key when a patient family members trying to sue the hospital with accusations of negligence after they realized that, there family member admitted in the hospital has a pressure ulcer on the sacral. However, this pressure ulcer was from home and the nurse has documented presence of the ulcer on the day of admission. The ulcer was documented its location, size and condition. The family members eventually noticed that the negligence was from home not from the hospital and they were forced to stop proceeding with their lawsuit.
