A year old female patient presented to clinical complaints of fatigue. It was noted her blood pressure was 201/105. The patient was allowed to rest throughout the visit. Medications were reviewed she was noted to take Inderal 40mg bid. The patient stated Inderal was prescribed for anxiety. The patient’s blood pressure was rechecked and was 195/98. The patient was administered Clonidine 0.1mg oral. After 25 minutes a repeat blood pressure of 170/94. The patient was diagnosed with CKD 3, per nephrology she was instructed to discontinue her Triamterene, given her Gfr in less than 30ml/min (Rubenfire, 2018). The patient was not given replacement therapy. The patient lives alone with no close contacts to help her. She states she did not take her blood pressure at home. The patient will see nephrology in 2 weeks.
The patient was prescribed Losartan. Losartan is an Angiotensin II Receptor Blocker (ARB). Losartan is not only used for hypertension but has (Mulla & Siddiqui., 2019) protective characteristic to the kidney. Losartan lowers the risk of stroke in patients with cardiomegaly and hypertension. Losartan is a second line guideline for hypertension
Close monitoring of electrolytes and renal function is imperative. Although we used Up-to-Date to find a medication to help treat, ARB’s can cause hyperkalemia. Preferably when the patient is evaluated by Nephrology she will be monitored and medication can be adjusted. The patient will go to Oncology tomorrow and they will check her blood pressure. She will return to clinic in 1 month for
re-e valuation.
Given the patients age and health instructions were reviewed both orally and written down. Patient understands the plan.
