My clinical reasoning skills have improved significantly throughout the FNP program. For example, I had a patient in clinical that presented with right leg pain. After performing a thorough HPI, I discovered that he had 14 back injuries throughout his life. I also discovered that the pain radiates laterally down the right thigh, to the back of the right leg towards the back of the right heel. Luckily, I looked up the dermatomes associated with his pain and made a diagnosis of right lower back pain with sciatica. The plan thereafter was to obtain an X-ray to rule out any abnormalities, refer to his prior chiropractic (as this helped him immensely in the past), and follow up in 6 weeks for his acute on chronic right lower back pain.
An evidence-based management plan for a patient with fibromyalgia is another example of taking into consideration what means the most to the patient. Her pain was significant and she had tried everything. On the follow-up visit, her fibromyalgia decreased significantly on duloxetine, and without looking up evidence that this medication works well for this condition I would not have known that.
Health promotion is a practice I enjoy utilizing for all patients. We had an elderly couple come into the clinic and I soon realized they were both dealing with their own physical and mental health problems. We set them up with a home visit/home care program, performed a nutrition assessment, and discussed home exercises they can do to stay active. As a physiological threat to health, frailty is a predictive factor of negative health outcomes, such as the increased risk of mortality, a decrease in daily life activities (ADL), higher hospitalization, physical limitations, falls and fractures, social withdrawal, and delusions (Silva et al., 2019, p 2).
There was a woman who came in for a pap smear that identified as a Muslim woman and she spoke Arabic. We were able to plan for a live interpreter visit. We learned from her that Muslim women prefer a female healthcare provider. She was due for a pap smear and we performed the exam, and she was very thankful that we were there for her. We also explained to her cervical cancer and why we screen when performing pap smears. Healthcare professionals need to be aware of privacy and touch issues when it comes to Muslim men and women, dietary practices, and unacceptable medicines (Attum et al., 2022, p. 1)
Interprofessional collaboration is essential when working in health care. An example of utilizing interprofessional collaboration in practice is when I would discuss the patients presenting problems to the provider and my preceptor. We would often come with up a plan for the patient together after I weighed in initially and saw the patient first. An example is when a patient that saw the provider came in and reported tremors in his hands. I discovered that the patient drinks 6 whiskeys a night. Thereafter I was able to score him on a CAGE assessment and determined he had a substance disorder. We decided together that the patient would benefit from Gabapentin to decrease anxiety and cravings for alcohol and he was deferred to behavioral health. Both pharmacologic and behavioral treatments have demonstrated efficacy for alcohol use disorder ( McHugh & Weiss, 2019, p. 1)
