Community nurses can play an important role on stroke prevention. Strokes play a major role in lifelong disabilities and mortalities, even though they have had major advances in treatments and diagnosis (Clare, 2017). There are many modifiable risk factors of stroke. Nurses play a pivotal role in providing information to patients in the community, hospitals, and primary care. There is education that can be provided regarding primary and secondary risk factors. Providing prevention advice to the community can assist with detecting and treating many modifiable risk factors in order to try to prevent a stroke from either occurring or reoccurring (Clare, 2017).
There are many health nursing interventional strategies that can be utilized when addressing the health concern that I will be discussing with the population at the assisted living in which I will presenting my educational project. I will be providing health promotion to the community. Hypertension is one of the main contributors and precursor to stroke. I will provide education regarding modifiable risk factors for hypertension, such as, obesity, poor diet, smoking, alcohol consumption as well as psychological stress. All these modifiable risk factors can play a role within each other. For example, you can become obese if you have a poor diet, are not active and do not exercise. Also, I will be discussing the importance of smoking cessation, as well as how alcohol consumption can play a role in increasing the risk of stroke.
It is also important to identify chronic conditions that can contribute to the co-morbidity of stroke. Some examples are atrial fibrillation, diabetes mellitus, carotid artery disease, and increased lipid levels (Clare, 2017). Teaching and providing self-management support to our older adults is also an important task that community nurses can take on. “However, at a broad level, self-management is about the day-to-day management of one’s own chronic conditions (Clark et al., 1991; Lori & Horman, 2003)” (Bolscher‐Niehuis et al., 2019, p. 195). Self-management of chronic disease process can also help decrease the risk of having a stroke. All these different disease processes are contributors and precursors to stroke. Self-management education is important even it is directed to the population in assisted livings, since they still have a lot if independence. It is important to educate them on early prevention of stroke, as well as the continuous treatment on chronic diseases that can contribute to the co-morbidities of stroke. Community nurses has many ways in providing medical education to the communities on the prevention of many different medical conditions. Nurses will always play a pivotal role on educating patients to try to maintain a healthy lifestyle and have a quality of life.

Focusing In on my topic surrounding developmental disabilities and health resources that are available to them for my Community Health Education Project. Growing older with a developmental disability is seen in the context of increased recognition of health disparities over their life course (Hahn, 2014). Interventions for this community to focus on health promotion, disease prevention, social support, and community- based physical activity to increase their quality of life to live life to the fullest of their disability.
Screening patients with developmental disability for other acute or chronic illnesses. Making sure these individuals have access to health insurance and health service such as medical, mental health, physical and occupational, supportive care, and or long-term care services to help meet the needs of their disability. Improving the communication helping nurses’ visibility on nursing intervention, studying outcomes, and education future nurses on how to properly care for those with different disabilities (Hahn, 2014). Focusing on health maintenance and reduction and intervening with educational programs focused on weight and nutritional management.
Healthcare education as the most frequently reported nursing intervention on both visits for a large majority reinforces the knowledge that individuals with developmental disabilities often rely on support to assist them in understanding preventative health measures and health behaviors directed towards good health (Hahn, 2014). Some of these individuals have support care staff and with a company that help assist them with medication, preventative care , and annual examinations so holding education competencies to help educate the staff on what to look for and how to help the patient maintain their help with show much success. Providing more in home or in facility preventative care measures so that the patients are comfortable in their environment and can be easily assessed without the need for transportation and pulling staff out of staffing to go to office visits. Healthcare professionals increasing communication between developmental disability clients being aware and understanding their behaviors to help ease the process of care received and provided. Conducting behavior management to those individuals with behavioral disparities to help the clients understand appropriate behavior to and for others. With all the interventions that were listed the major goal is to promote health and prevention of further disease, so that healthcare improves the quality of care for individuals with developmental disabilities.
