PART 1
During Consultant Chat #2, you will collect information about your Consultant and use that information to create a Medical Emergency ID Wallet Card. THE MEDICAL ID CARD IS TO BE GIVEN TO THE CONSULTANT AND IS CONFIDENTIAL BETWEEN YOU AND THE CONSULTANT.
1. Full Name
2. Address
3. Email (YES, some older people really DO communicate via email!)
4. Phone Number
5. Date of Birth
6. Physician Information (name, phone)
7. Emergency Contacts (first and last name, phone, relationships)
8. Medications / Supplements ( name, dosage & frequency, reason for taking it-existing medical condition)
9. Allergies and reactions
10. Insurance Provider(s)
11. Other Information (advanced directive, blood type, preferred hospital)
After collecting the above information:
1. Use a piece of card stock paper to print out the Medical Emergency ID Wallet Download Medical Emergency ID Wallet card . (attached below)
2. Enter your consultant’s information on form provided.
3. Be sure to leave the completed card with your consultant.
PART 2
Consultant Chat #2: Scripted Questions (These are for your journal entry!):
Ask the following questions and put them in your journal entry: IN THE JOURNAL, PLEASE LIST THE QUESTION FIRST FOLLOWED BY THE ANSWER.
1. Did you receive a completed Medical Card from your student? Do you think it will be useful to you?
2. How do you pay for medicine? Is the expense for your medications a burden on your budget?
3. Do you go to the pharmacy to get your medications or have mail order delivery? Did you know there is a law that says that a licensed pharmacist MUST do an education session with every patient who comes to a pharmacy to get a prescription filled?
4. How well does your insurance cover the cost of your prescriptions? Is the expense a burden on your budget?
5. How have the changes in Medicare affected you?
Consultant Chat 2 Journal Entry
1. Collect the medical information for questions 1- 11 in Part1 above and record on the Medical Emergency ID Wallet Card and give to your Consultant.
2. Post the answers to the scripted questions 1-5 in Part 2 in your journal entry.
3. Remember to include your critical reflections at the end of the journal entry for full credit.
Post what you have learned from your consultant in the journal entry of at least 250 words (not including the questions) for credit.
