Question 1
Laura is a 25-year-old school teacher who, on the recommendation of a friend, attends your clinic reporting that she has developed right sided lower back pain. She tells you that she previously saw a chiropractor for mid back ache when she was a teenager which felt better with treatment and she also had sessions with a physio for the same mid back ache.
Due to the pandemic she helped to set up a lot of online resources in her school but was one of the teachers who continued to teach face to face with small groups of key- workers’ children.
Whilst she tried to keep as active as possible during the lockdowns to keep up her fitness levels (normally she attends classes in a gym) she feels that she may have gained some weight in spite of this as she reports that her ‘tummy feels bigger’.
She takes a daily multivitamin, recently stopped taking Microgynon and occasionally uses a ‘blue inhaler’.

Discuss how you would explore this patient’s case giving a rationale for further questions you would ask, consideration of flags, evaluations you could undertake and the possible hypotheses based on the information given. Decide whether you would refer. If you do, what investigations would you like to be carried out? Discuss the rationale for your osteopathic treatment and management plan for Laura.
Section 2: Imaging (20%)
What type of imaging is it? Describe what you can see and how might it affect a patient’s health and function.
Section 3: Pharmacology (25%)
What is (a) Microgynon and (b) a ‘blue inhaler’? Why would Laura be prescribed these drugs? What are the actions of these drugs? Briefly discuss the main side effects of using these drugs.
Question 2
John is a 72-year-old semi-retired architect who attends your clinic reporting an 8-month history of intermittent stiffness and pain in his right lower back, hip and knee. He puts this down to an increase in walking over the lockdown period when he began looking after his son’s young dog. He takes on occasional work projects which means liaising with clients and visits to building sites to check progress. Currently, he is involved in a complicated and challenging project which is giving him ‘a few sleepless nights’ but he says he still enjoys doing this but is finding it a little difficult to climb ladders on site.
John reports that he gets up in the night to go to the toilet but thinks it is the ache in his back and leg which wakes him. He says he feels well, has a good appetite and finds it easy to get back to sleep when he gets back into bed.
He hasn’t seen his GP since March 2020 as his GP hasn’t returned to face-to-face appointments yet. John currently takes Atorvastatin having previously been prescribed Simvastatin.
