Background
You are the supervisor of three nursing units in a hospital that just opened last year. It is now one year later, and the units have been open for one full year. You are looking at the HCAHPS (Hospital Consumer Assessment of Healthcare Providers and Systems) Scores for your units for the last 4 quarters (year) to see if you can determine a pattern. The data was collected by the administrative team where you work from surveys administered to patients upon discharge.
You have data for Questions 1-9 for your units by quarter. (See questions below.) You want to do something on each unit to raise the scores, but you need to analyze the data first to see what the root causes of any issues are. You decide to put this data in a spreadsheet to better analyze the information by calculating the average (mean) score for each question for each unit and by calculating the average (mean) score for each quarter for each unit.
Your goal is to implement an intervention tailored to each unit’s needs to raise the scores.
All labels should be centered.
All numeric values should be centered and with one decimal place to the right of the decimal point (e.g., 3.2).
See the “Spreadsheet Example” below. The sample contains different data and formatting; therefore, be sure to follow the specific directions provided here for your spreadsheet.

Create a spreadsheet
Make a spreadsheet in Excel of the data provided where the quarters are the rows and the columns show you the questions and center your column labels in the cells (Be sure to label the questions as Q1, Q2, Q3, etc. NOT 1, 2, 3, etc.). Be sure that you include a cell label for “Quarters”. (Seen in the spreadsheet example).
Subdivide the questions and label them as the following: 1-4 are labeled as “Nurse Questions”, 5-7 are labeled “Physician Questions” and 8 & 9 are labeled “Environment Questions”. Label this row as “Category”. Center this label.
Input the mean score data into the Question/Quarter cells and center the data in the cells
Calculate mean values (center these values within the cells and choose a format that uses number format and one decimal place.)
Use the formula function in Excel to calculate the mean score for each unit for each quarter row
Use the formula function in Excel to calculate the overall mean of the question columns
Use the formula function in Excel to calculate the average for the unit overall.
Apply cell attributes: Bold and italics
Apply the bold attribute to all labels in the first column to the left, all labels in the top 3 rows, and all calculated mean values (i.e., Question, Quarter, and Overall means).
Apply italics to the labels for each question category (i.e., Nurse Questions, Physician Questions, etc.).
Apply cell shading and border lines
Apply dark grey shading to the cells with labels in the first row from the top (unit names) and the individual cells that contain the overall unit mean (bottom right cell for each unit).
Apply lighter grey shading to the cells with labels in the second and third rows from the top (i.e., “Category/Question Category” and “Quarter/Questions” rows).
Apply a single, thick line as the outside border.
Apply single, think lines for all inside borders.
Apply font size
Change the font to Arial 14 point for the first row from the top (unit names) and the individual cells with the labels for “Category”, “Quarters”, and “Mean” in the first column to the left.
All other cells should be Calibri 11 point.
Create 3 bar charts
Create a bar chart in Excel for each unit showing the mean score for each individual question only (not the overall mean)
Be sure that the correct variable names appear on the X-axis (e.g. Q1, Q2, Q3, etc.).
Be sure that the Y-axis has the same range for all three charts.
Change the indicator bars to a distinct color for each unit (Yellow – Oncology Unit, Orange – Cardiac Unit, Green – COVID – 19 Unit).
Properly label each axis so the X and Y titles are clearly identified as “Mean Score” and “Questions”.
Add a title to the top of each chart (within the chart frame) to show the name of the unit.
