For this assignment, you will use the assessment data for John Smith that is provided as well as the BASC assessment results. ***Following the example in your text on pages 405-406***, you will CREATE all the criteria needed to complete the report EXCEPT the scores that you are provided.
Your report should include a thorough Reason for Referral, Background Information, Observation in both the classroom and test session, a Summary of diagnostic impressions, and Recommendations. You should use all the data to formulate your diagnostic impressions and you should be examining if a significant discrepancy exists between John’s ability and achievement that would indicate a specific learning disability [WRITER: You may choose any learning disability you believe fits the data]. How does the data reflect John’s response to the interventions that have been attempted?
