This assessment report requires no *cited* sources, BUT the following sources must all be used in detail: [1] Behavior Assessment System for Children (BASC-3) Results (John Smith’s mother, JANE SMITH, took this assessment, please report accordingly), [2] Wechsler Intelligence Scale for Children (WISC-V) report, [3] Woodcock Johnson IV report, Assessment Procedures for Counselors And Helping Professionals Textbook Ch. 17 Pages 402-407. All of these resources are attached, as well as a rubric. Please refer to the rubric carefully to make sure all criteria are met. School Counselor will complete an assessment report using information from Chapter 17 (pages 402 – 407) as your guide. For this assignment, you will use the assessment data for John Smith that is provided as well as the BASC assessment results. ***Following the example in your text on pages 405-406***, you will CREATE all the criteria needed to complete the report EXCEPT the scores that you are provided.
Your report should include a thorough Reason for Referral, Background Information, Observation in both the classroom and test session, a Summary of diagnostic impressions, and Recommendations. You should use all the data to formulate your diagnostic impressions and you should be examining if a significant discrepancy exists between John’s ability and achievement that would indicate a specific learning disability [WRITER: You may choose any learning disability you believe fits the data]. How does the data reflect John’s response to the interventions that have been attempted? Consider if based upon the data provided whether or not they would qualify for Special Education Services [WRITER: please recommend John for Special Education Services with support from the data]. Your recommendations should help address the presenting problem of the child based on all the information gathered in your assessment report. Make sure you provide a thorough rationale for your recommendations.

Student demographic information:
The date of the assessment is the date that you completed the BASC-3 Rating Scale. Please list yourself as the Evaluator.
Reason for Referral:
Include educational grades and records, regular benchmark test scores, and student progress.
Background Information:
Parent, teacher, student interview concerns, attempted classroom interventions, and consultation.
Behavioral Observation:
Provide information from classroom observation and test session observation.
Assessment Instruments, Results, and Interpretation:
WISC-V, WJ-IV, BASC-3
Summary and Diagnostic Impressions:
Use all of the data to formulate your diagnostic impression based on the analysis of all of the assessment info. Do significant discrepancies exist between ability and achievement?

Your recommendations should include concrete treatment objectives based on evidence-based interventions.
