For each of the 10 community resources, you should do the following on this template:
•    Name of the community resource or community organization
•    Contact information for that community resource
•    Note the resources or references you used to find the information (ex: website, flyer, pamphlet, interview, etc.)
•    A description of the individuals served by that organization (ex: disability, age or developmental level, gender, etc.)
•    A short summary of the services provided and links summary to the course content (ex: unique needs of individuals with emotional disabilities); this can be narrative or bullet pointed
•    If you don’t complete all 10 annotations, full credit is not received
-Final Reflection
•    In the same document, reflect on this activity and what you have learned. This should constitute 4-5 concise paragraphs. These questions should guide the reflection:
o    What did you learn?
o    How did your experience support (or not) the information covered in the course?
o    What implications do your findings have for you as a teacher or professional working with students with emotional disabilities?
