This project will be the creation of New Telemedicine Care Model for Chronic Disease Management. Specifically mental health in the pediatric population. This will need to be specific to Arkansas. Either option requires inclusion of the following items at a minimum: The issue you are trying to solve Real industry data that supports the need for a solution (analytics or scholarly articles) Stakeholders needed from the provider community Stakeholders needed from the technical community Stakeholders needed from the reference data or content management community Social Determinants of Health and discuss their influence on pediatric health and outcomes. Partnerships required (vendors, community services, other providers, patients/caregivers, advocacy groups, etc.) Technology(ies) that will support the model Include discussion on interoperability and electronic exchange of information Include discussion of data governance Existing and currently proposed legislation/regulations that will have an impact on the model (this can be direct or indirect impact) Reimbursement considerations A diagram of the workflow Scripture or personal Christian views that drive your choices in the creation of your model 7-10 pages (can include 1 page of graphs or charts).
