Case study Mary Brown is an 80-year old widow living alone on her farm 10 kilometres from town in a western rural state. The town, with a population of 10,000, has a critical access hospital. Mary lives in the farmhouse to which she moved when she married 60 years ago. She would like to stay there as long as possible, but after her hip replacement surgery last autumn, her mobility is not as good as it used to be. She is mildly hypertensive and is on medication. Her type 2 diabetes is under control with diet and medication. Mary has lots of friends in the community, but her grown children live in other states. Because of her isolation and age, she might be considered “at risk”. Case Study 19.2, taken from Larsen (2016) p.474 Write an essay critically analysing the following: What are Mary’s “at-risk” variables? Which self-management tasks should be initiated with her? What does Mary need? What are her potential needs? This is a small community with few resources. Be creative and develop a model of care for her. In order to answer these questions you should: Read the case study carefully Identify the issues involved What theories of chronic illness would assist your understanding of these issues in this context? What recent research illuminates these issues? How are the issues in the case study related to the questions posed? In what context do the problems arise? Family situation? Services available? Financial considerations etc? What does the literature say about possible solutions to these problems? How do you evaluate these solutions to find the best solution for this context? Your essay should have: a title page with your name, student number, unit code, unit title, assignment number and title, submission date and your word count; an Introduction that provides a context and clearly states what you will be writing about; subheadings in the body to provide structure and focus; a Conclusion that clearly demonstrates how you have addressed the key points; APA formatted in-text citations and reference list that clearly acknowledge all of your sources.
