Background
As a PMHNP, benzodiazepine use in practice for the treatment of disorders of anxiety, sleep, and adjunct therapy to antipsychotic medications, are amongst a few reasons why patients seek the use of these each day. Knowing how to manage the use of these medications is necessary, especially when it’s in the best interest of your patient.
Instructions
For this exercise, you will assume the role of the PMHNP managing a patient with an anxiety/trauma-related disorder who is currently on a benzodiazepine but there is a need to taper the patient as you seek to establish a new treatment using a non-addictive medication. Based on the case study you receive, you will provide the following information as you seek to care for your patient.
Part I: Description of the Case
Patient demographics, educational level, and cultural affiliation
History of present illness
Part II: Medication History and Assessment
Medication history, including an adherence assessment
Pattern of changes, tolerance, and reported side effects
Part III: Patient Adherence Education
Based on the identified pattern of medication changes, tolerance, side effects, and adherence assessment, provide your patient with methods by which these can be improved, providing this at the appropriate educational level, and inclusive of the patient’s cultural affiliation.
Part IV
Create a table for tapering your patient from the benzodiazepine in the case file.
This table should showcase the details of what the patient should take every day, including the milligram dosage for the medication you are tapering. If your case file calls for you to also advance a medication, you will need to show that on the table as well. This should show substantial detail so as to be able to clearly follow the medication taper from one day to the next until the patient has been transitioned off.
Provide the patient with specific psychoeducation so as to identify important side effects the patient might see when tapering and threshold for seeking additional help.
Students should use APA formatting to detail your findings in this exercise, including use of the APA Manual 7th edition title page, page numbers, double spacing, in-text citations, and references.
