Module 6: Activity G – Learning from Incidents and Human Error This activity requires you to design and prepare a summary information sheet on a case study of an incident that involved human factors. Part of your reading for the Unit and preparation for this Activity is to read the Step Change for Safety document “Human Factors How to take the first steps..”accessible at https://humanfactors.atlasknowledge.com/resources/Human_Factors First_Steps.pdf Your Activity should use the same format and headings as the twelve Case studies (p7-18) illustrated in the document i.e. • Case Study Title • What happened? (Brief succinct description) • What human factors were involved ? • What did people do intentionally? and/or • What did people do without meaning to? • Which Barriers were involved? (note under each heading) • What can we learn from this incident? You should not be able to determine where the incident occurred. You may choose an incident that has occurred in your current or previous organisation, or one from an on-line site such as HSE, or industry – you can search accident case studies on website. Your submission should be no more than 500 words. 2. Module 6: Activity H – Human Factor Analysis Use the template proforma below to record a human factors analysis of a chosen task within your organisation (or a task you understand well enough to use . maintenance task). This proforma is in the document Human Factors: Inspectors human factors toolkit on p41 of 114 (refer to for a completed example). You will have to create a simple table to complete and upload, as you prefer . within word or excel. Although word count is not a critical factor with this activity, it should still be manageable within the normal word limit expectation of 500-750. It is the process of understanding and reading around the topic and applying the material to a summary assessment of the task that is being evaluated. Do not feel you have to write 750 words.
