Complete an assessment of the Foster Family using the principles of Calgary Family Assessment Model (CFAM) and a strengths-based philosophy.
Hypotheses Phase: Prior to the family interview in week 3, each student is to review the scenario information on Brightspace and complete the Shadow Health focus exam chest pain. In the Shadow Health online program students will have the opportunity to collect subjective and objective information related to the case. Based upon this information, each student will generate 2-3 hypotheses for the concept map and care plan. In other words, based on the information provided, what do you anticipate the care needs of the family to include?
Interview Phase: Information obtained during an interview with the Foster Family will be posted on Brightspace on Jan 27
Analysis Phase: Each student will complete their own analysis of the data collected and the family interview based on their chosen hypothesis and interview data collected.
Planning and ImplementationPhase: Based on the individual student’s analysis of the information collected/provided, each student is to create a concept map and a care plan focusing on possible changes that would support this family’s capacity within the chosen area. The concept map can be computer generated, neatly hand drawn, or a combination of both. Although one page (letter, legal or ledger) should be sufficient to create the map, an additional page can be used to further explain the work or provide extra details, if required (for example, your hypotheses). The care plan should be no more than three pages (excluding cover page and reference list). The care plan may be developed using the template posted on Brightspace OR it may be presented in text and point form. (See template posted on Brightspace).
Evaluation Phase: Each student is to provide evaluation criteria as to how the family will demonstrate success in meeting the expected outcomes.
