A. Simulation Scenario Assignment
• Students will request a simulation care setting from list of available settings per faculty
instructions.
• Faculty will attempt to provide a patient care area as requested by students; however, cannot
guarantee availability of requested care area. Scenarios will be assigned on a first come, first
served basis.
• All simulation scenarios are unique to the assigned student.
• Faculty will assign a student-specific simulation scenario by providing a Word document with
the simulated setting description, measure data-the opportunity for improvement-and
patient care area details.
• Following submission of part 2 of the project paper, final post-implementation data will be
provided to the student by faculty for completion of part 3.
B. Simulation Scenario Project Presentations
• Presentation forums will be assigned by faculty.
• Students will develop and present a Microsoft PowerPoint video presentation per criteria and
uploaded in the assigned forum. Instructions for upload will be provided in Canvas.
Presentations will be limited to 6-8 minutes.
• As part of a community of learning, all students (presenters and peers) will engage in a
scholarly discussion of projects.
• Peer students will view and provide a substantive scholarly reply to two (2) students in
assigned forum per criteria. One or more scholarly references and citations are required.
Directions for PROJECT & PAPER:
Research the simulation scenario problem. Perform a literature search for scholarly articles through
the CSUF Library as well as locating credible websites (e.g., Centers for Disease Control &
Prevention) to gain a full understanding about the simulation scenario problem. This is an essential
foundational step in the quality improvement project learning activity.
Research the approaches to solve the problem and achieve the aim. In part 2 of the project,
students will again conduct a literature search to identify two evidence-based approaches to solve
the problem. The approaches must be aligned with and be able to solve the problem and achieve
the aim. An interdisciplinary team approach is highly recommended and/but any approach must
include a strong nursing presence in the approach. Education may not be a selected approach;
rather, education is an element of the action plan to implement the approach.
Please use the headings listed in the TEMPLATE for organizational clarity.
Your summary paper will include scholarly research with seven to 10 scholarly references in
APA format. Begin your reference page and use of scholarly references with Part 1 and add to it
as each part is submitted.
• The total paper should be 10-13 double spaced pages, not including title page, reference list,
and appendix.
• Senior level university writing standards are expected including format, grammar and
punctuation, spelling, and style. Scholarliness of paper, including proper use of APA is expected.
Points will be deducted for lack of scholarliness including excessive use of quotes and improper
paraphrasing, or poor writing or issues with APA or references. The writing and references
section is graded with each part of the paper.
• Turnitin© will be utilized for the evaluation of originality of the work product, adequacy of
paraphrasing, overuse of direct quotes, and plagiarism.
QUALITY IMPROVEMENT SUMMARY PAPER TEMPLATE:
1. Part 1 Introduction (about ½ page) (No header). Introduce the clinical setting (e.g., Emergency
Department, L&D, Telemetry, Clinic), the problem and its significance to the clinical setting and
population served. Include 1-2 scholarly references to demonstrate the significance of the
problem.
2. Part 1 Background (2 pages) (Header, bold, center). Develop a scholarly discussion of background
information about the problem including the causes and contributing factors and significance of
the problem. The discussion must demonstrate a full understanding of the problem and integrate
the scholarly references. Describe the current state of the problem, including data, that supports
the need for improvement and include at least 3-4 scholarly references (e.g., current patient falls
at your organization are higher than the national average, current HCAHPS scores are lower when
compared to national data, current CAUTI rates for the simulated unit that demonstrate an
opportunity for improvement (problem) that is significant compared to national rates or the CDC
goal for CAUTI reduction). Comparison data (if available) should be referenced. Demonstrate the
current state data by creating a graph. For example, if your problem is CAUTI, graph the national
rates and rates for your facility. Your data should be briefly discussed and referenced. You will use
this graph to compare your data at the end of the project. Attach graph in your Appendix. Place
the Appendix following the reference list.
3. Part 1 Problem Statement (Two sentences only) (Header, bold, center). Succinctly state the
problem that will be addressed. For example, “Despite low or no cost immunizations for
vulnerable children served at the Family Clinic in Orange County, childhood immunization rates fell
well below Healthy People 2020 target rates. The current system of documentation and
recordkeeping system does not provide a reliable process to remind parents and clinicians when
immunizations are due.”
4. Part 1 Aim Statement (One sentence only) (Header, bold, center). Identify the aim of the
planned change: what are you going to improve, by how much, by when. For example, “the aim of the proposed planned change is to improve childhood immunization rates (the what), from
45% to 75% (by how much) by Nov. 10, 2019 (the when).” Be sure you can measure your results
by the due date of the final paper. The aim statement must be aligned with the background and
problem statement. Use the following format: “The aim of the proposed change project is to
[increase, decrease] the [number, rate] of [problem] from X to X by [date]”.
5. Part 1 Change Theory (about ¾ page) (Header, bold, center). (Paragraph 1) Consider different
change theories and select a change theory that will serve as a framework (foundation) for the
action plan for implementation of the selected approach (Part 3). Discuss only the selected
change theory. Describe the change theory. Provide a description of the stages of the change
theory and briefly state the activities that occur in each stage of the change theory. Two or
more scholarly references are required. The course text may be utilized as one reference.
6. Part 1 Graph (Appendix Letter, bold, center). Place following the reference list. Provide
baseline data in a graph to be used to compare results at the conclusion of your change project.
7. Part 2 Approaches to Solve the Problem (about 2 pages). (Header, bold, center).

Perform a literature search to identify evidence-based approaches to solve the problem and
achieve the aim; the evidence must be integrated into the discussion of each approach. The
approaches must be linked to the problem and demonstrate the ability to solve the problem.
Consider two different evidence-based approaches to solve the problem. Identify, describe, and
discuss two dissimilar* approaches and provided evidence that the approach is effective (e.g.,
50% reduction in number of falls with harm) to solve the stated problem and achieve the aim.
Discuss at least one potential problem with each approach. The approaches must be nurse-
centric and/or interdisciplinary approaches. Education, in any form or method, will not be
accepted as a primary approach to solve the problem (Zero credit/points); however, may be
included as part of the action plan. Medical practice/physician-focused approaches will not be
accepted for credit (Zero points).
*Approaches must be different (dissimilar). For example, two educational approaches
are not considered different.
8. Part 2 Analysis of Potential Success (about 3/4 page) (Header, bold, center). Begin with a simple
straightforward statement identifying which approach was selected; do not mention or discuss
the non-selected approach. Describe the care setting factors that you think will help your project
be successful (facilitate the successful implementation of the approach) AND barriers (restraining
forces) that will inhibit the successful implementation of the approach.
9. Part 3 Quality Improvement Project Action Plan (about 3-4 pages) (Header, bold, center).
Describe your action plan within the context of the selected change theory and how you
implemented your project (simulated action plan). Include activities and responsibilities for
implementation (e.g., leadership, interdisciplinary team, CNS, etc.) and the evaluation plan
associated with the approach (e.g., process measures). Use two or more quality improvement
(QI)/performance improvement (PI) tools such as but not limited to a cause & effect (fishbone)
diagram, driver diagram, or workflow analysis (algorithm) as part of your implementation plan.
Describe and discuss use of all QI/PI tools in the action plan. Place QI/PI tool(s) in the appendix.
Your QI/PI tools will be evaluated for content and correct application of the tool.

errors). DO NOT simply refer to a graph in the appendix. Place graph in the appendix.
Analyze the data (paragraph 2). Compare your results to baseline data and external comparative
data (e.g., California or National benchmarks if included in scenario) and your aim (e.g., percent
reduction in falls with harm). Compare to the external benchmark (if available) and the project aim.
11. Part 3 Evaluation (1 pages) (Header, center, bold). Evaluate your project success or failure,
(simple straightforward declarative sentence). Evaluation must include factors that enhanced the
achievement of the aim (driving forces) and factors (restraining forces) that detracted from
achieving the aim. Identify factors (driving forces) that would have supported implementation.
Discuss strategies implemented to mitigate the barriers (restraining forces) from achieving the aim.
12. Part 3 Conclusions (1/2-1 page) (Header, center, bold). Formulate a succinct conclusion about your
project by restating the problem, the approach implemented to solve the problem (not a
restatement of results), implications and applications to practice.
13. Part 3 Recommendations (1/2-1 page) (Header, center, bold). Formulate recommendations for
improvements and next steps to optimize, expand, eliminate aspects of the approach and/or spread
the approach. Create recommendations in the form of declarative explicit statements—proposals—
for the next steps or actions to take (e.g., “My recommendation is…”). Ambiguous inexplicit
statements do not meet expectations.
14. References (Header, center, bold). The paper must include seven to ten (7-10) scholarly
references in APA format. Please note: Organizational references and product brochures and
marketing materials do not count as a scholarly reference.
15. Part 3 Appendix. (Header, center, bold). The appendix must include at a minimum three graphs: 1)
pre-implementation baseline data with comparative and/or targets; 2) process measure data
(student-developed based upon the selected approach), and 3) post-implementation data to clearly
demonstrate the results of the quality improvement project compared to baseline and external
benchmarks. Include all the QI/PI tools (e.g., cause & effect diagram, driver diagram, workflow
analysis) that you used in the appendix of your paper. Graphs and QI/PI tools must be correctly
formatted per standards provided and labeled (APA format). Graphs and QI/PI tools are linked to the
problem statement, aim statement, and selected approach. All content placed in the appendix must
be on a separate page under a specific appendix letter.
