This paper is to be in APA format, with correct spelling, punctuation, and grammar. The reference page is to include all references used in preparation of this paper. At least 1 journal article (less than three years old) is to be included. Medications, labs and vital signs need to be listed in table format as shown. Paper is due on assigned date, maximum of 6-8 pages should be sufficient.
Diagnosis: Review causes for renal failure. Pick a significant cause and write a short narrative history. This will be a patient you have seen previously or one you have developed for this project. Hint: Please research GFR and determine what decreases are a concern, what is GFR range in ESRD? Explain the pathophysiology relating to the cause of this patient’s renal failure and why he will now need dialysis.. Is the patient a candidate for peritoneal dialysis? How would you determine if they were a candidate or not? Is the patient a candidate for a kidney transplant? Why or why not?
Diet/Fluid Therapy: Include type of diet and fluid restriction seen with a dialysis patient, explain rationale for these dietary and fluid changes. Compare renal failure (RF)patient diet to end stage renal failure (ESRF) patient, are they the same? Would compliance be a problem? Does patient have urinary output? How much urinary output is normal? Use a nutrition resource for information.
Medications: Discuss all medications the patient is taking and why, including Erythropoietin. In addition to nursing actions, include any special considerations for dialysis patients. Individualize this information, taking into consideration the interaction of the medications with one another, as well as potential problems resulting from interaction of the medications with diet, fluid restriction, lifestyle, and other considerations. We have listed the top medications, please add other medications you have seen in practice or you come upon that you feel should be on the list. What does a Renal Vitamin contain?
Do any of these medications dialyze out? Name a medication that dialyses out.
Any need to be given after dialysis instead of prior too? Why would you give after?

4.Vascular Access Site: Explain the type of site being used and its advantages and
disadvantages compared to other types of access. (Fistula vs. Graft)
5.Frequency: Include the length and frequency of normal dialysis sessions.
6.Dialysis: Explain how dialysis works. Include a discussion of how the composition of
the dialysate solution is determined, and how this influences the dialysis process.
Three broad types of dialysis are peritoneal, Hemodialysis, and Continuous Renal Replacement Therapy (CRRT). Briefly describe the type of patient each would be recommended for and how they are different.
7.Complications and Interventions: List complications that a patient might have following a dialysis session or during the session? Explain cause of complications and how treated.
8.Psychological: How does hemodialysis affect a patient psychologically? What types of resources may they need, what restrictions does it put on everyday living?
9.Lab and other data: Be sure to review lab values that reflect pre-dialysis and post-dialysis sessions. Explain deviation. What types of radiographic studies would your patient have had previously? List at least two.
