Identify the community agency that you chose. How does the agency serve this vulnerable population? What is the mission of this agency in relation to the vulnerable population? What are the services provided for the vulnerable population? Why did you choose this agency? Assessment/Analysis: Describe the process of assessing a community: Explain the 2 different pieces of community assessment: community reconnaissance and the Key Informant survey. What community resources did the Key Informant state were available for use by this agency? What other resources did the Key Informant state would be useful to have, but to their knowledge, are not available in the community? Sum up the Assessment/Analysis phase in the paper by using your Data Analysis Tool to give an overall picture of the community. What were the “good” things (protective) and what were the “bad” things (predictive) that stood out about the community? Discuss prioritizing a problem list with the help of the Key Informant (KI). Explain how you narrowed down the list to the top 3 problems that you and the KI felt you could do something about. How vital was the input from the KI?
