A 44-year-old female patient presented to the office this week with complaints of hair loss, sweating, and increased anxiety. She was also wondering about medications to take for weight loss. She has a significant PMH of anxiety disorder with moderate depression.
I provided health promotion this week by promoting and encouraging the patientâs lifestyle changes of her recent exercise program of strength training and cardio that she performs 4 times per week. I also guided the patient toward different medications to take for weight loss. In the clinic I am at, we use Wellbutrin and Naltrexone for weight loss. The patient was interested in this medication, side effects were reviewed, and it was prescribed for her.
After discussing her increased anxiety, she was in agreement to re-initiate the psychiatric referral and a referral was placed to a provider closer to where she lived ( <1 hour) which will significantly improve her chances of going through with the referral. We also discussed her family. She is a single mom of four children, which can significantly influence her health. She mentioned she tends to put the needs of her children in front of her own. We discussed small ways to put her health first and she was in agreement to try and focus more on her health for her children. Social determinants of health (SDH) evoke factors such as health-related features of families (e.g., relationship with a partner, family size, and accessibility of healthful foods), which can influence health-related behaviors (Braveman & Gottlieb, 2017).
Psychotherapy, in association with pharmacotherapy, is associated with better efficacy and better outcomes for patients with anxiety/depression ( Thibault, 2017). Finally, we discussed taking her 50 mg Hydroxyzine up to four times per day for acute anxiety instead of at night prior to sleep as she was doing previously. I utilized health promotion by asking the patient what she does for self-care- we discussed meditation and deep breathing for causes of acute anxiety and the patient perceived these recommendations well.
We also talked about how decreasing her stress can have significant improvements in her hair loss. Nonscarring alopecia can be readily diagnosed and treated in the family physician's office. Androgenetic alopecia can be diagnosed clinically and treated with minoxidil. We prescribed her Minoxidil which she will use for four months ( Phillips et al., 2017).
