Health care reform and NP practice
With the passage of the Affordable Care Act in 2010, more individuals can access affordable health care coverage and increases the need of primary care providers to meet this need. NPs are well positioned to fill this need in their clinical excellence. Politicians and nurse practitioner interest groups are framing the need for NP full practice authority across the states to meet the demand for qualified primary care providers. Nurse practitioners can be part of the workforce to meet the increased demand for provider. Throughout the research, NPs continue to demonstrate superior care equitable to that of a physician, improving health outcomes, safe, and cost-effective care (Brom et al., 2018). Addition, I feel the parts of the ACA was specifically designed for NP practice. The ACA aims to improve equitable and fair patient care, preserve Medicare, improve health prevention and promotion, and establish new care models (Hoyt & Proehl, 2012). In some ways if feel the ACA poised an unbeknownst challenge to APRN practice in that its a âlet us see what we can do with thisâ. For example, Medicare preservation, NP practice is more affordable than that of physician counterparts. Health promotion and prevention is the very foundation of nursing philosophy and is aligned with NP practice. Finally, establishment of new models of care, in that nurses managed care clinics far exceeds the new model goals.
NP reimbursement under the affordable care act
Our assignment in unit two was indeed an eye opener for me, in that how NP practice restrictionâs impact not only the NP profession, but also health care quality and health care costs and goes against the aims of the ACA. Rural states with NP practice restrictions result in care delay, poor health outcomes, and decrease health as primary care offices may not accept Medicaid and employ NPs who have a lower Medicaid reimbursement compared to their physician counterparts. Medicaid is now the largest fee-for-service reimbursement within the US. Higher reimbursement from state Medicaid is improves health care and outcomes for those beneficiaries and in turn provides a cost savings in health outcomes, fewer inpatient hospitalizations due to unmanaged chronic conditions, and fewer use of ER visits for primary care health needs. Improving Medicaid reimbursement rates incentivizes primary care providers to accept Medicaid patients and employ NPs (Barnes et al., 2017).
My experiences
I myself do not have any personal experiences with the impact of the ACA affecting NP practice. My experiences are only those I have researched and studied in my program. If anything, I have a desire to work in a Federal funded clinic. I feel I will be more rewarded and professional fulfilled working in this environment than that of a private for-profit practice.
