Upon completion of this assignment, the student will:
1. Demonstrate the ability to synthesize and apply knowledge related to a chosen area of nursing practice.
2. Integrate findings from best practice to describe the current state of nursing. Knowledge relevant to identified problems, interventions, prevention of complications and holistic nursing care.
Student will develop and create a patient scenario that describes at least 2 health Issues, student will develop the patient’s background and comorbidity’s.
Introduction:
• Brief description of the clinical issue and its significance to nursing and healthcare ( Identify two specific patient problems)
• Based on patient problems utilize best practices from nursing literature.
Case Description include:
• Health problem
• Medical determinants
• Social determinants (cultural, economic, educational, institutional factors)
• SBAR
Proposed Holistic plan of care, for each identified patient problem.
Include details of level of care, interventions, potential risks, and complications and rational for the following:
• Management of Care
• Safety and Infection Control
• Health Promotion and Maintenance
• Psychosocial Integrity
• Basic Care and Comfort
• Pharmacological and Parenteral Therapies

Discuss the Role of the nurse in relation to:

Development of Patient Case Scenario (20pts)
Must have at least 2 health issues – A SBAR and related physical assessment findings are included
Organization of paper; Grammar/Writing Style: (10 pts)
APA format and Paper is logically organized:
Paper proceeds from introduction to summary of patient and health issues
All sections of paper outline are discernable
proper use of headings; professional writing style (no jargon, etc.); Ideas should be logically presented; No typos / spelling errors present.
Synthesis and Analysis (60 pts)
Based on Current Standards of Nursing Practice student will develop a patient scenario/case study including relevance of all interventions/ management plan, social factors relevant to the case and the role of nurse are addressed. Achievement of client outcomes should be guided by the over-arching domains of Safe and Effective Care Environment, Health Promotion and Maintenance, and Psychosocial Integrity, Physiological.
