Team-Based Primary Health Care Initiative
The purpose of this team assignment is to develop a primary health care community-based initiative to address a public health population-based issue, identified by your team, which employs coalition building, relational development with policy leaders, as well as technology for education and/or outreach. This assignment builds on the work you did in your individual assignments, and must include the assigned readings from the text as well as additional readings assigned throughout this course. The team will share in the one grade.
Each team will make a presentation to the class on their focal topic and their proposed initiative. The class will act as an interested party, and the team will have to ‘pitch’ their idea in a convincing way to entice a potential ally. For example the group may be trying to induce a stakeholder organization to join the coalition; lobby for support from a politician for necessary legislation; or procure funding from an philanthropic organization.

a. Identify primary health care initiatives in the United States that had been developed, and implemented to address the population-based public health issue that this team selected.

1. Give the social, political, and economic context of the initiative, and the time frame in which it operated.
2. Were nurses/was nursing involved in this initiative? If so, describe the nursing role.
3. Explain how the initiative did/did not address issues of cultural congruency, economics, and social justice.
4. What were the outcomes of the initiatives?
5. What kind of technology was used within these initiatives?
6. Evaluate the strengths and weaknesses of these initiatives. What worked? What did not? Explain why.
7. What kinds of collaborative relationships were developed? Did a collaboration or coalition emerge to address the target phenomenon?
b. Discuss the legislative context exists with regard to this population health issue.
i. What governmental organizations oversee this area of health?
ii. What are the major laws governing the management of this health concern?
iii. What funding streams are directed toward addressing this health concern
