Planning the Intervention: Discuss the community health nursing diagnosis, goal, and objectives for the clients that you created from your analysis of the assessment and the problem list you created. Explain the process of how you chose the particular interventions. Offer at least 2 valid rationales for why you chose the specific interventions. Describe and explain the evidence-based intervention(s) that you created. Discuss what evidence-based information/research supports the intervention that you created. Evaluation Did the Key Informant understand the intervention and how/when to use it? (use the results of the Key Informant evaluation—did they meet your objectives?) Did the Key Informant understand how to use/administer the evaluation tool? Discuss how this project helped you: Do you understand and feel that you know the community better? The vulnerable population better? Find the “Needs Assessment” that the hospital at which you are employed did for their community (if you are not employed at a hospital, use any hospital in your community). List the link and give a brief discussion of the areas that were assessed, plus the conclusion/recommendations. Did you identify any resources in your community resource assessment that might help with any of the needs identified by the hospital? Maybe they overlooked these resources—or maybe these resources didn’t exist when they did the Community Needs Assessment. If so, what were they?
