Step 1: Summarize personal history.
Describe how each of the six topics below relate to your personal experience.  Your personal health history should include six paragraphs on your childhood, one paragraph for each of the following topics:
Illnesses (accidents, injuries, etc.)
Prevention (dental, medical)
Family health patterns (exercise, hygiene, etc.)
Family eating patterns
Drugs and alcohol in the family
Stress factors that affected your sense of wellbeing
Note: I understand that these six topics may include very sensitive information, so I want to encourage you to only share what you are comfortable.  If you feel a topic is too sensitive, feel free to focus on a different experience within that topic.  You still need to discuss the general topic, but you can control the focus.
( there is an example in files )
Step 2: Identify practices to continue or change.
To conclude your personal health history, identify a few practices/habits you want to continue from your childhood and which practices/habits you want to change and why you want to keep or alter these practices.
Step 3: Submission.
Submit a 2-3 page, type-written, double-spaced, 12-point serif font (i.e., Times New Roman) paper.  Submit paper with a cover page in APA style
My English is so bad but this is an example of my life health. You can add more and of course you need to fix all of the wrong words and make it more clear in your own words
During my childhood I didn’t have many accidents, I was a girl whose parents overprotected me a lot. but I did suffer from a lot of illnesses and
The reason is because I suffered from asthma, this is one of the reasons why my parents overprotected me a lot, I could hardly go out to play and if I did go out I always had to be warm. Almost every week I had to be at the doctor to receive medication or injections.
When I was growing up my disease was reducing thanks to the treatments I was taking.
When I was 11-12 years old I did have many accidents like a motorcycle accident where I broke my nose and another where I broke my foot.
My mother was very demanding with our health and education every 6 months we had to go to the dentist.
I had a lot of cavities and I used braces for 5 years, that was a lot of years because I never followed the treatment as it should have been. My mother always fed us very well, she cooked for us every day.
My adolescence was very difficult. I had a disease called anorexia which took me many years to get out of it. It all started because people always told me that I had a very nice body, that I was thin with a very small waist and that I looked like a model because of how tall I am. There was not a day that all the people told me how beautiful my body was and that it was enviable. I liked to eat, but I wanted to maintain that body that everyone envied and that’s how my anorexia began. I ate a lot and then vomited to eat again and vomit again. At the end of the day I only drank more than 5 liters of water and chewed gum to calm my anxiety. My mother always cried to see me bad. She weighs almost 80 pounds. And she was emaciated. Thanks to the family that helped me get out of this disease and made me understand that she was not looking at me as pretty as I was before suffering from this disease.
Over time I got better and I went back to my regular weight what I used to have  before. It took me a long time to regain my weight, but I managed it, although it’s not the same, but I learned to eat and lead a healthy life.
In my family as sisters, father and mother there are no problems with drugs or alcoholism, but on the part of my uncles and grandparents my 2 maternal grandparents died of alcoholism and my 3 uncles too.
My mother and I suffer from migraines, it is something I have learned to live with.
Some things that I would like to continue to have in my personal life is to continue eating well and thanks to the help of my mother that may be possible because, although she is a little older, she continues to worry about our health and she still is cooking for us
