This case-based discussion is on a cancer patient who attended the assessment unit with a seven-week history of a cough, two-week history of haemoptysis and intermittent chest pain radiating across his right side. My primary differential diagnosis in this particular case was PE.
As a nurse practitioner, I am always very cautious when treating patients with suspected PE as people with cancer have a higher risk of developing blood clothes. The investigation for a PE is a CTPA, but the patient in question asked me why CTPA and no VQ scan.
In my critical reflection, I will investigate the evidence around which modality would be the most effective in obtaining a diagnosis for PE CTPA vs VQ scan??
