16.1 Quality Improvement Plan This week, you will develop quality improvement plan based on the responses to your interview during week 14 and throughout the course. This plan must be focused on a clinical, organizational or administrative goal in your healthcare organization. The plan must include the following; Your team (Who will be on your improvement team and why?) Your aim. Provide a clear and detailed description of the quality issue. What changes need to be made? WHY? What changes will be made that will lead to improvement? Your baseline measurement? Where are you now? What will you measure that will be indicative of success or change? How will you test the change? (ex: PDSA Cycles, other process analysis tools) How do you plan to implement the change? Maximum 10 pages – not including title page, abstract, references or appendices. Minimum of 4 academic and/or professional references. Hospital websites, internets, magazines or other non-professional or non-academic resources do not qualify as scholarly sources. Include the appropriate APA citation for personal communications. Remember, personal communication citations are not included in the References section of your paper, only in the body of the text.
