PURPOSE
The goal of this assignment is to submit the Community Health Assessment Worksheet Form. This form should include detailed data regarding the selected community of interest.
The Interprofessional Collaborator (IPC) will work with the student to help identify the community of interest and complete the Community Needs Assessment. Additional collaboration will take place as the community action plan is implemented and evaluated.
