The QI nurse, with the help of the new unit manager, developed a standardized shift-to-shift hand-off report with a one-page tool to be used for each patient. The intent of this tool is to provide a comprehensive, structured approach to providing critical information on new and recovering postoperative patients. The new tool was introduced by the new manager at a monthly staff meeting and was scheduled for immediate implementation. At the next monthly staff meeting the nurse manager reported that only a third of the nurses were using the tool consistently. Most of the nurses voiced that the tool was time-consuming and rarely completed by the time the report was given.
