The delivery of health care is one of the most regulated sectors in this country. There are overlapping layers of federal, state, and local government laws and policies that govern where, when, how, and who should deliver care. Yet, as reports from IOM and AHRQ have shown, there are persistent gaps in delivering high quality consistent with the six dimensions of healthcare quality(Agency for Healthcare Research and Quality (AHRQ), December, 2020; Anderson, Roundtable on the Promotion of Health Equity the Elimination of Health, & Disparities Board on Population Health Public Health Practice Institute of Medicine, 2012). Affordable Care Act also added more incentive programs to improve quality with demonstratable results(Figueroa, Zheng, John Orav, Epstein, & Jha, 2018). However, AHRQ’s recent reports indicate that ACAâs policies have not eliminated medical errors or improved the quality of care to marginalized communities. The major question, therefore, is are federal and state policies and laws really able improve healthcare quality? Why or why not? If yes, how and why hasnât worked already?
