As a community health nurse, I was shocked by how much Community Health Nursing 1 taught me. Then again, I had the same awestruck experience in Community Health Nursing 2! Being able to actively engage with a different target audience than usual within my community was a wonderful opportunity to see numerous health determinants I had not originally known about and engage with members I would never have engaged with prior. I am a pediatric nurse and my target audience for this entire course has been older adults and elderly. I am grateful to have been able to learn about the content in this course not only for my personal everyday use but also to be able to create the educational project with a focus on heart disease prevention and overall improved health!
I feel as though I was able to use technology to access previous community health assessments and learn demographic information for my county of focus. I was then able to use technology to advance the educational levels on heart disease within my target audience. I feel as though my presentation was helpful for the community. There were numerous members of the presentation who reached out post presentation via e-mail to ask follow up questions or receive the list of resources available within the community. It is my hope that the eagerness to learn and resource availability will continue to create generational health. Obviously though this presentation was not something I could have done alone so I am grateful for the connections I have standing previously within the community and the connections I’ve made for this project. creating professional partnerships is a large part of community nursing but also nursing in total. The interpersonal relationships a nurse makes with the patient is vital, just like the professional connections the nurse makes with other health care professionals to better serve the patients. A nurse must adapt to a level of teamwork that can best serve the patient in clinical decision making (Molina-mula et al., 2017).
I do feel as though I did an excellent job on meeting the objectives and turning in my assignments in a timely manner. Although on the back half of this degree, I do feel like I have finally found a time management system that works well with working full time as a nurse and meeting objectives in my college courses. Which is something I’m super proud of for myself to have done, finally!

Community Health Nursing II has made my knowledge base of the community health nurse grow significantly. I was able to develop and implement a health project. I was able to present this to a target audience in the community. It allowed me to grow in my field of nursing. This course also taught me how to gather information, learn more about health disparities, and collaborate with community partners to assist in improving population health, and present educational information to my target audience that allowed health promotion. This course also allowed me to also learn about the professional standards more in depth.
I was also able to incorporate several essentials from the baccalaureate essentials located in the AACN faculty tool kit (AACN, 2009). As I incorporated essentials II into my process of implementation of my project, I realized that I was working on quality improvement of patient outcomes, as well as inserting my knowledge in my leadership style. As I was preparing for my health presentation and narrowing down my topic, essential III, I performed this essential by incorporating latest evidence based practice in my research and implementation of my presentation. I wanted to be able to provide my target audience with the newest information on the prevention of stroke.
The other professional standards that were outlined in the syllabus (Essentials VI, VII, and VIII), where also incorporated in this course. This was achieved by the oral health presentation that I delivered to my target group on stroke at the assisted living. I was able to collaborate with other pertinent team members in order to narrow down my target group, topic and facility of the presentation was performed. I was able to communicate and deliver preventative health information to my audience using evidence based date, professionalism and professional values (AACN, 2009).
Conclusion, I surprisingly really enjoyed this course. It has made me realize that nursing is utilized in such a vast amount of ways clinically, as well as in the community. There are still many underserved and disparities in the United States. For this reason we must not ever forget to also take care of this population. Nurses can make such a difference in these communities even just by simple education on health prevention and promotion. Nursing will always play a significant role in health promotion and assisting patients to have a good quality of life.
