I know these surveys and goals are personal, but you can use your own case to answer the questions. For the introduction part, the name you can leave it as xxx, same with the major as well (if you need it). Also for the group meeting part, you can ramdonly make up 2 dates, such as Feb 1 at 1pm and March 20 at 1pm, the group member name can be xxx as well.
